About the role:

As a Case Manager, you’ll be at the heart of helping individuals recover from injury or illness. You’ll liaise with medical professionals, employers, and clients to develop personalised plans, offering support and guidance every step of the way.

Your responsibilities will include:

  • Managing a caseload of clients, ensuring they receive the support needed to achieve their recovery goals.
  • Collaborating with medical professionals to design and implement rehabilitation plans.
  • Maintaining regular communication with clients to monitor progress and make adjustments as necessary.
  • Building strong relationships with key stakeholders to facilitate smooth recoveries.


About you:

We’re looking for someone who has a genuine passion for helping others, combined with strong communication and organisational skills. You’ll also bring:

  • Experience and Qualifications: A background in health, rehabilitation, or case management is ideal, and a relevant qualification in allied health, nursing, or social work will be highly valued.
