He kōrero whakamārama mō tēnei tūranga | About the role
As an Advanced Support Worker at Access Community Health/Uru Ātea, you will work closely with the care coordination and community nurse teams, with indirect supervision from the supervising clinician. The role involves conducting non-complex client evaluations, as directed by the local Care Coordinator Team Leader.

You will also provide complex client care and carry out advanced tasks, for which you will have received training and competency verification from your clinical line manager. Additionally, you will take on a leadership role, offering on-the-job and just-in-time training to less experienced support workers, helping to develop and strengthen the SW workforce.

Ngā tohungatanga mō tēnei tūranga | About you

  • Positive, can-do attitude and experience in behaviour management is a plus.
  • Proficient in Microsoft Word and Excel (training provided).
  • Able to work effectively within multi-disciplinary teams (e.g., Palliative Care, Home-Based Nursing, Support Worker Coordination).
  • Must have a full or restricted New Zealand driver's license.
  • Level 4 Health and Wellbeing Certificate (or equivalent), or Level 3 with a plan to complete Level 4.
  • Career force Assessor Qualification is a plus.
