To coordinate home-based care and support services that enable Access clients to live as safely and as independently as possible in their homes and communities.

Nga mohiohio e pa ana ki tenei mahi | Key Responsibilities

  • Coordinate Home-Based Care: Oversee care and support services to ensure clients live safely and independently.
  • Build Relationships: Develop strong connections with team members, clients, and whanau (family) to foster trust and communication.
  • Deliver High-Quality Service: Ensure care plans and services are delivered to the highest standards, in line with policies and procedures.
  • Manage Support Workers: Coordinate placements to ensure optimal care and client satisfaction.
  • Adapt to Client Needs: Respond flexibly to the unique and changing needs of clients within a structured framework.
  • Complaint Resolution: Lead the team in addressing and resolving client complaints, escalating issues when necessary, and finding effective solutions.

  • Experience in team management, staff training, and rostering.
  • Strong interpersonal skills with the ability to build relationships across diverse groups and resolve conflicts.
  • Comfortable with technology (basic computer skills required; full training provided).
  • Excellent organisational and time management skills with a keen eye for detail and accuracy.
  • Ability to work under pressure, multitask, and adapt to changing priorities.
  • A proactive attitude and willingness to learn.
  • An interest in healthcare and community services.
  • Understanding and respect for cultural diversity, including a willingness to learn about Te Ao Māori values.
  • A commitment to putting Whānau first and ensuring that the needs of the family are prioritized in service delivery.
