He kōrero whakamārama mō tēnei tūranga | About the role You will play a crucial role in ensuring that our clients receive comprehensive and coordinated care, which is especially important for those with chronic or complex health conditions. This includes scheduling and managing appointments with various healthcare providers to ensure timely and appropriate care.
Takohanga | Responsibilities
Oversee the rostering of complex care clients and their support teams
Participate in Support Worker training, ensuring adherence to sector standards
Build strong relationships with complex care support teams and clients
Assist with training of the complex care teams
Identify and support recruitment needs of the identified complex care clients
Ngā tohungatanga mō tēnei tūranga | Skills and experience
Experienced in long term conditions, aged residential care or medical care
Intermediate level computer and on-line information recording skills
The ability to plan and organise caseloads effectively, to establish priorities and meet deadlines in a demanding, client focused environment
A current full driver's license is essential
A demonstrated commitment to Te Tiriti o Waitangi in the workplace
Actively demonstrates the ability and desire to show cultural sensitivity, awareness and understanding of diversity
