Join our team as a valuable partner to Primary Care GP Practices and NGOs in Northern Wellington. At Kelburn Medical Centre, you'll collaborate with Health Improvement Practitioners to assist patients with mild to moderate health needs. Enhance community support access, facilitate choice, and work closely with the team to identify the best-suited providers or agencies. Our service provides short, targeted interventions to individuals.

This full-time role is working Monday to Friday between the hours of 8.30am to 5.00 pm in Wellington.

As a member of our Wellington Pathways' Access and Choice team, you'll be under the guidance of the Team Coach. Collaborating with a dedicated group of Support Workers, your role will involve working within the community. Utilizing your expertise as a skilled practitioner, you'll assist individuals in setting goals and implementing action plans. Strong connections with local GP practices would be advantageous in this position.

We are interested to hear from people who have a caring nature and work in a recovery focused way.

  • You must hold a current full driver’s license.
  • Hold a current first aid certificate or be willing to obtain one.
  • Hold the New Zealand Certificate in Health and Wellbeing (NZCHW) or a recognised equivalent.
