The Role | Mō tēnei tūranga mahi
0.5 FTE, Wednesdays and Fridays from 8.30am to 5pm, Thursdays half-day to be agreed, and based in Nelson.
The Access and Choice service provides free health and wellbeing support to people registered with their GP practice. The health coach works collaboratively with health professionals at the practice to improve health and wellbeing outcomes on a wide range of presenting issues particularly where lifestyle change can make the difference. The health coach works on the GP premises, in the Pathways office, or can make community visits.
In this role, you will:
- Work across the lifespan, supporting individuals of all age groups.
- Collaborate with and serve as a resource person for the general practice team.Demonstrate strong cultural awareness to effectively support our diverse community
You:
- You must be eligible to work in NZ, which includes having NZ residency or a valid NZ work permit.
- Hold a New Zealand Certificate in Health and Wellbeing (NZCHW) or a recognised equivalent.
- A satisfactory police check is required as part of the employment process.
