We have an exciting new opportunity now available for a Registered Health Professional who is keen to work permanent part-time hours as a Clinical Coordinator in Tairāwhiti

About The Role | He kōrero whakamārama mō tēnei tūranga

This is a part-time permanent role working 16 hours per week. No shift work or evenings!

This role will be based across the Tairāwhiti region. You must be comfortable travelling across this area.

As a Clinical Coordinator/Registered Nurse, the successful candidate will be responsible for introducing Geneva Healthcare and obtaining consent to plan and provide Home and Community Support Services to support and empower our clients and whanau. You will be implementing initial Service Plans and ongoing service plan reviews based on funder requirements and when the client has changes in their health status or care requirements.

Responsibilities include but are not limited to:

  • Maintaining the delivery of services according to multiple contract specifications and adherence to Geneva’s bicultural strategy and commitment to Te Tiriti o Waitangi
  • Working collaboratively with other health professionals across various teams to support timely recovery and rehabilitation of clients aligned with goals
  • Identifying, growing and maintaining community linkages / key contacts and stakeholders while promoting Geneva Healthcare services
  • Providing integrated clinical support across services by identifying needs and responding accordingly
  • Provisioning of clinical advice as required to community support workers enabling them to provide safe and efficient care with clinical supervision by means of clinical competency training and assessment through skill sharing
  • Risk management, de-escalation and problem solving in a professional and efficient way while maintaining excellent customer service
  • Undertaking of clinical work as required

What you will bring | Ngā tohungatanga mō tēnei tūranga:

The ideal candidate will demonstrate the following:

  • A registered health professional with a current practising certificate
  • Proven skills in health assessment, planning and goal setting
  • A willingness to develop new skills in service promotion and marketing
  • A full Drivers Licence
  • Demonstrated ability to work collaboratively with other professionals and the disability sector to maximise health outcomes and goal achievements for client’s and their families
  • Confidence in managing IT applications and the ability to report regularly on services provided and outcomes achieved to a proficient level
  • Proficiency and ability to provide hands on clinical nursing cares i.e. catheter cares, wound care and various other clinical tasks in the community to a range of clients
