About the role | Mō tēnei tūranga mahi
Are you the healthy lifestyle advocate we are looking for? Someone who is passionate about supporting people on their recovery journeys? If so, then keep reading.
This is a full-time position working 40 hours per week. You will be working Monday – Friday, 8am – 4:30pm. Based in Hamilton.
As a Healthy Lifestyle Coordinator, you will be kept busy supporting people experiencing mental illness with their individual recovery and wellbeing plan by providing activities-based support.
You will have a leadership role in the team, supported by your Team Coach and oversee the review of new referrals to the service. You will have a natural ability to build and foster great partnerships with both internal and external stakeholders, including but not limited to other Pathways services, our local mental health clinical partners, whanau and community supports.
Day-to-day will include supporting a small case load of 1:1 referral with goals around healthy lifestyles e.g. fitness/movement, nutrition, health awareness, spirituality, social connection and work readiness. You will regularly plan and facilitate healthy lifestyle group-based activities within our Pathways services and in collaboration with external stakeholders. As a team you will be responsible for planning and facilitating monthly wellbeing events to the wider Pathways Auckland region alongside the Pathways Healthy Lifestyle National Calendar. Woven through these supports will include creating wellbeing resources for the people we support, their whanau and our kaimahi.
The Team
You’ll be reporting to the Team Coach and joining our Pathways Community Health and Wellbeing Team. Each team member brings their own unique skills and passions to this role which makes it a fun and creative environment! The main focus of our team is to help people experiencing mental health challenges to develop goals and skills to manage their wellbeing through healthy lifestyle choices.
We understand that everyone has different needs, so we tailor our support, and help to plan and navigate journeys to physical and mental wellbeing through personalised and responsive one-in-one support, regular engagement in group events and activities..
You | Mōu
We are interested to hear from people who have a caring nature, are passionate about helping people on their recovery journeys and have a passion for improving their overall health outcomes. You will be proactive, being able to use your initiative and think on your feel and be a good problem solver. You will be able to work well as a team as well as autonomously manage your caseload and day-to-day commitments.
You will ideally also:
- You will have knowledge of health and wellbeing programmes and how to develop and implement these
- You’ll have the ability to present, facilitate groups and develop and run programmes based on health and wellbeing
- You will have experience in organising and facilitating activities and or events
- You’ll be a great listener, the sort of person who will listen deeply to the people who use our services. You’ll be energising, inspiring others with enthusiasm and a will be working well in a team
- Hold current full driver’s licence.
- Hold current first aid certificate or be willing to obtain one
- Applicants must hold the New Zealand Certificate in Health and Wellbeing (NZCHW) or a recognised equivalent. Alternatively, you will commence study towards this within six months of starting your employment with Pathways. This needs to be completed within two years of commencement. Pathways offer significant support for support workers to complete this study.
