The role | Mō tēnei tūranga mahi Nelson's Community Mobile team supports tāngata whai ora with mental health and addiction challenges to thrive and achieve wellbeing.
The role is primarily focused on providing medication support to people living in their own homes in the community. The role also provides some community support, where the focus is on supporting tāngata whai ora to thrive independently. The support is a meaningful connection taking an holistic approach to wellbeing.
This is a permanent role working 32 hrs per week. You’ll work Friday to Monday on a weekly rotating shift pattern of 7am-3.30pm and 1pm-9.30pm, based in Nelson.
You will be:
Providing medication support
Making a meaningful connection (eg checking in on their day and their plans, and how they are)
Supporting wellbeing (keeping an eye out for their physical as well as mental health)
Keeping in touch with the care team (about how they are and if there are any medication related issues to be discussed)
You | Mōu We are interested to hear from people who have a caring nature and work in a recovery and strengths-focused way. You will enjoy supporting people facing mental health and/or addiction challenges. We value people who have lived experience of mental health and addiction challenges themselves and are prepared to use their lived experience to support those on their own journey of recovery.
