About the Role | He Kōrero Whakamārama Mō Tēnei Tūranga

As a Huntington's Disease Service Coordinator, you will play a vital role in supporting individuals diagnosed with Huntington's disease and their whānau. Your focus will be on ensuring seamless access to health and disability support services while coordinating care to enhance their quality of life. This includes face-to-face engagement, needs assessments, and collaboration with healthcare professionals to ensure appropriate interventions are provided at the right time.

Key Responsibilities:

  • Coordinate services and respond to client needs promptly.
  • Organise and participate in HD Specialist Clinics 4-5 times annually at the Christchurch Brain Research Institute.
  • Conduct assessments and develop person-centred shared plans in collaboration with healthcare professionals.
  • Provide information and guidance to individuals and their whānau regarding HD progression.
  • Maintain accurate and up-to-date client records and service utilisation data.
  • Establish and maintain strong relationships with clients, whānau, and healthcare professionals.
  • Ensure compliance with contractual obligations, health sector standards, and company policies.


This role covers the Canterbury region, including Kaikōura, Hurunui, Waimakariri, Banks Peninsula, Selwyn, Christchurch City, Ashburton, and the Chatham Islands.

What You Will Bring | Ngā Tohungatanga Mō Tēnei Tūranga

To be successful in this role, you will need:

  • Registration as a health professional (e.g., nursing, occupational therapy, social work, or physiotherapy).
  • At least 3 years of post-graduate experience or relevant life experience.
  • Strong understanding of health and disability sector standards.
  • Experience working with individuals experiencing long-term illness or trauma.
  • Excellent interpersonal, verbal, and written communication skills.
  • The ability to prioritise workload, work autonomously, and meet deadlines.
  • Proficiency in database management and IT systems.
  • A full NZ driver's licence and access to a reliable vehicle.
