SilverdaleMedical is seeking an experienced Nurse Practitioner (NP) with a passion for chronic disease management to join our team. Key Responsibilities:
Chronic Disease Clinics: Lead best-practice management for patients with Diabetes, Heart Disease, Lung Disease, and Obesity.
CollaborativeCare: Work as part of a multidisciplinary team with GPs, nurses, pharmacists, and Health Improvement Practitioners to optimize patient outcomes.
Patient Empowerment: Educate and support patients in making sustainable lifestyle changes, improving adherence to treatment, and self-managing their condition.
Evidence-Based Practice: Implement clinical guidelines and work at the top of your scope to ensure high-quality, proactive care.
Clinical Leadership: Support the practice in advancing chronic disease care models, contributing to quality improvement initiatives.
Requirements:
Registered Nurse Practitioner (NP) in New Zealand
Minimum 3 years of experience as an NP with significant exposure to chronic disease management
Strong ability to work autonomously while integrating into a collaborative primary care team
A passion for preventative medicine and patient education
Experience in lifestyle coaching and motivational interviewing is an advantage
